  INSURANCE INFORMATION 
It is important that you understand that most insurance agreements require that you authorize me to provide the insurance company with a clinical diagnosis. Once this information reaches the insurance company it is part of your file and will be maintained under their jurisdiction. If you have any questions regarding confidentiality of information given to your insurance company please consult one of their representatives. Also see the Confidential and Privacy Policy section on this website as well as the HIPAA policy for additional information regarding your rights.
 
I am an in-network provider for the following insurances (I am contracted with each of them) and will bill them regularly on your behalf. I will need a copy of your identification card or all of the information contained on that card forwarded to me prior to our first appointment or at our first appointment. I will confirm your coverage with you either via email prior to our first session if time permits or when I have confirmed your coverage but as early as possible.

Patients covered by these insurances are requested to pay the designated co-payment defined by their insurance policy and any remaining deductible at the beginning of each session. The insurance company will pay the remaining session allowance directly to me and you will receive a notice from them each time a payment is made.
 
Insurance Reimbursement when Therapist is out of network or a PPO Provider
For all other insurance plans not included above, I am considered an out-of-network provider or a PPO. I will still bill your insurance company on your behalf and to do so I will need a copy of your identification card or all of the information contained on that card forwarded to me prior to or at our first appointment. I will confirm your coverage with you. This will allow you to decide if the financial information fits your situation before we meet in a first face-to-face session. There can be an applicable deductible amount and/or higher co-pay than if a member chooses a contracted therapist. In this situation, payment for the full session is to be made at the beginning of each session; any payment by the insurance company will be made directly to you.

$90.00 Individual Session
$100.00 Couples Session
$125.00 Family Session
Unless other arrangements are made, it is my policy to request full payment at the beginning of a session

Cancellation Policy

If you do not show up for your scheduled therapy appointment, and you have not notified me by telephone at least 24 hours in advance, you will be required to pay the full cost of the session. Please note that insurance will not cover missed sessions
